About applying to register your gender
Registering your gender will make sure you have the proper gender listed on your identity documents.
There are different registration forms and processes for people under 16.

What you need to apply
Applying for yourself
You must provide:
- a completed application form (PDF, 2.2MB) including:
- personal contact and information details
- the gender you are registering as
- proof you are over 16
- a gender declaration statement, witnessed by a Justice of the Peace (JP) or a Commissioner for Declarations
- reason for applying to register gender
- proof of identity documents
- evidence of counselling (if applicable)
- payment for the fee.
Applying for a child under 16
You must provide:
- a completed application form (PDF, 2.2MB) including:
- personal contact and information details for you and the child
- the child's gender declaration statement, witnessed by a Justice of the Peace (JP) or Commissioner for Declarations
- the parent(s)/guardian(s) declaration statement, witnessed by a Justice of the Peace or Commissioner for Declarations
- reason for applying to register gender
- evidence of the child's counselling (if applicable)
- proof of identity documents for parent(s) or guardian(s)
- payment for the fee.
Can I apply?
To apply to register your gender, you must
- be over the age of 16
- have undergone counselling (if applicable)
- have your birth registered in Tasmania.
If you are applying for a child under 16
The child must:
- be under 16
- be a consenting party
- have undergone counselling
- have both parents completing the application
- have their birth registered in Tasmania.
One parent can apply for a child if:
- they are the only parent on the birth record
- one parent is deceased
- the guardian is the only guardian of the child; or
- the registration of gender is approved by a magistrate.
Proving your identity
There are 2 ways you can prove your identity:
- show your original identity documents at the Births, Deaths and Marriages office, or
- send certified copies of your identity documents by post or email.
You must provide one of these combinations of current documents:
- One document from category 1, 2 and 3
- Two documents from category 2 and one from category 1 or 3
- Two documents from category 3 and one from category 1 or 2.
At least one document must show your current address.
- An Australian birth certificate
- Citizenship certificate
- Immicard
- New Zealand citizenship certificate together with passport
- New Zealand birth certificate.
- Australian driver licence
- Australian passport
- Firearms licence
- Foreign passport
- Proof of age card
- Working with vulnerable people card.
- Medicare card
- Australian marriage certificate (issued from Births Deaths and Marriages office)
- Centrelink or Department of Veteran Affairs card
- Security or crowd control licence
- Tertiary education institution ID card
- Credit card or bank statement
- Recent utility account with current residential address (issued within last 3 months)
- Aurora+ phone application (name and address details must match the applicant's and be visible when sighted at a service centre. Must not be a screenshot)
- Lease or rent agreement with current address
- Australian Taxation Office assessment with current address.
If you have changed your name, please provide proof (from the list below) of each name change. This is so we can link your current name to the name recorded on the birth record.
Please provide one of these documents:
- name change certificate(s) issued by Births Deaths and Marriages
- marriage certificate(s) issued by Births Deaths and Marriages
- divorce paper issued by court (showing previous name)
- statutory declaration (if changed by common usage)
- relationship certificate issued by Births Deaths and Marriages.

- If you send your original birth certificate in with your application, we will send you an updated copy.
- You can only register your gender once in a 12-month period.
- If you post your application every identity document must be certified.
