  • After registering your change of name you will receive a certificate which is valid for all legal and official purposes, including proof of identity.
  • You must supply a valid reason for the change.
  • You can apply to change your name once in a 12-month period, and 3 times in your lifetime.

    About applying to register a change of name

    You must supply a valid reason for the change. Births, Deaths and Marriages are required by law to ensure that any change of name is not for fraudulent or improper purposes.

    You do not need to apply to register a change of name if your name has changed by marriage and you have an Australian marriage certificate.

    What you need to apply

    You must provide:

    • a completed application form (PDF, 403KB) including:
      • personal contact and information details
      • your new name
      • a valid reason for the name change
    • proof of identity documents
    • payment for the fee.

    Can I apply?

    You must be:

    • 16 or above
    • born
      • in Tasmania, or
      • interstate but have lived in Tasmania for 12 months or over, or
      • overseas but have obtained an Australian citizenship or permanent residency and lived in Tasmania for 12 months or more.

    Proving your identity

    You must prove your identity by providing:

    • identity documents to the Births, Deaths and Marriages office with your application (by appointment)
    • certified copies of your identity documents by email or post. To certify a document, you need to take the original and a photocopy to either a Justice of the Peace or Commissioner for Declarations to be certified.

    Identity documents

    The documents you need depend on where you were born.

    If you were born in Australia

    You must provide your birth certificate and:

    • two documents from List 4 (if you were born in Tasmania).
    • one document from List 3 and two documents from List 4 (if you were born elsewhere in Australia).

    If you were born overseas

    You must provide:

    • one document from List 2,
    • one document from List 3, and
    • two documents from List 4.

    • Australian citizenship certificate
    • Australian passport
    • Australian visa and overseas passport.

    These documents must show that you have lived in Tasmania for 12 months.

    • Bank statement (name and address page only)
    • Lease or tenancy agreement covering at least 12 months
    • Rates notice
    • Utility accounts (including gas, water, electricity, mobile)
    • Aurora+ phone application (name and address details must match the applicant's and be visible when sighted at a service centre. Must not be a screenshot)  
    • Department of Taxation group or advice certificate
    • Department of Veteran Affairs or Centrelink Pensioner concession card -including healthcare cards.

    These documents must be current and show your current name, with at least one showing your signature:

    • Australian photo driver licence
    • Australian passport
    • Government issued working with vulnerable people card
    • Tasmanian Government personal information card
    • Defence force or police service photo ID card
    • Medicare card
    • Credit card, account card, passbook account statement
    • Student ID card
    • Department of Veteran Affairs or Centrelink Pensioner concession card -including healthcare cards.

    Please note that you cannot use the same identity document twice.

    • If you send your original birth certificate to Births, Deaths and Marriages with the application, they will exchange it for an updated version showing your new name.
