- You can legally change your name by registering it with Births, Deaths and Marriages, Tasmania.
- After registering your change of name you will receive a certificate which is valid for all legal and official purposes, including proof of identity.
- You must supply a valid reason for the change.

About applying to register a change of name
You must supply a valid reason for the change. Births, Deaths and Marriages are required by law to ensure that any change of name is not for fraudulent or improper purposes.
You do not need to apply to register a change of name if your name has changed by marriage and you have an Australian marriage certificate.

What you need to apply
To apply online you must:
- complete the online registration including:
- personal contact and information details
- your new name
- a valid reason for the change
- verify and upload your proof of identity documents (please ensure you have a clear, readable scan or photo of ID. Front and back required for some ID)
- a valid credit card to pay the fee.
Please note, if you remain inactive for 30 minutes after starting the application, you will need to start again.
Can I apply?
You must be:
- 16 or above
- born
- in Tasmania, or
- interstate but have lived in Tasmania for 12 months or more, or
- overseas but have obtained an Australian citizenship or permanent residency and lived in Tasmania for 12 months or more.
Proving your identity
You must prove your identity by uploading your identity documents to the online application portal. You maybe be able to verify some documents through the Document Verification Service during your online application.
Identity documents
The documents you need depend on where you were born. Documents must be current (not expired).
If you were born in Tasmania, you must provide:
- A standard birth certificate issued by Births, Deaths and Marriages Tasmania scanned and ready to verify or upload.
- Two current identity documents from the list below in your current name, with one showing your signature:
- Australian photo driver licence (front and back)
- Australian passport
- Government issued working with vulnerable people card (front and back)
- Tasmanian Government personal information card (front and back)
- Medicare card
- Credit card, account card, passbook account statement
- Student ID card
- Department of Veteran Affairs or Centrelink Pensioner concession card - including Health Care cards.
- If you are a prisoner or detainee, you have obtained the consent of the Director of Corrective Services.
If you were born in another state/territory in Australia, you must provide:
- A standard birth certificate issued by the state or territory Births, Deaths and Marriages you were born in scanned and ready to verify or upload.
- One document from the list below which shows at least 12 months residency in Tasmania:
- Bank statement - (name and address page only)
- Lease/tenancy agreement covering at least 12 months
- Rates notice
- Utility accounts (includes gas, water, electricity)
- Australian Taxation Office group/advice certificate
- Two current identity documents from the list below in your current name, with one showing your signature:
- Australian photo driver licence (front and back)
- Australian passport
- Government issued working with vulnerable people card (front and back)
- Tasmanian Government personal information card (front and back)
- Medicare card
- Credit card, account card, passbook account statement
- Student ID card
- If you are a prisoner or detainee, you have obtained the consent of the Director of Corrective Services.
If you were born outside of Australia, you must provide:
- One document from the list below which shows proof of your permanent residency or citizenship in Australia:
- Australian citizenship certificate (front and back)
- Australian passport
- Australian visa and overseas passport
- Immicard
- One document from the list below which shows at least 12 months residency in Tasmania:
- Bank statement - (name and address page only)
- Lease/tenancy agreement covering at least 12 months
